I will say that Antibirth actually has more content and changes the game more spectacularly than Afterbirth+, and is certainly worth playing in its' own right - however, I do feel Afterbirth+ has added things that thematically make sense, as in it fits into the lore of the game, whereas Antibirth (with things like mine levels, which really don't fit the game) does not do as well.
